1. Florida Residents
   
   Permanent Parking Permit (4-Year): The applicant, or his/her parent or guardian, must complete HSMV form #83039 (if DMV database reflects a certifying authority date within last 12 months of replacement date, a new form is not required), HSMV form #83146 and provide a Florida driver license or identification card.
   
   Temporary Parking Permit: The applicant, or his/her parent or guardian, must complete HSMV form #83146 and provide a Florida driver license or identification card.



2. Out-of-State U.S. Citizens
   
   Temporary Parking Permit: The applicant, or his/her parent or guardian, must complete HSMV form #83146 and provide an out-of-state/U.S. Territory driver license or identification card or U.S. passport.



3. Foreign Citizens


Temporary Parking Permit: The applicant, or his/her parent or guardian, must complete HSMV form #83146 and provide a copy of a current out-of-country parking permit (both sides) and a passport or comparable identification.


4. Organizations
   
   Permanent Parking Permit: The applicant must complete HSMV form #83146.
