Parking Permit Requirements for Foreign Country Citizen

DHSMV Form 83039 must be completed by theapplicant or his/her parent or guardian.

A copy of the out-of-country parking permit. (both sides)

A passport or comparable form of identification.

Payment of fees. (Temporary Parking Permits Only)
